What to Expect on Your First Visit
Your first visit to The Cottage is the most important. It is during this time that you will get a chance to sit down with
the doctor and discuss what is most concerning to you regarding your health. You will have the opportunity to have
a private, confidential, one-on-one conversation with the doctor. During this conversation, you will be asked to
discuss your current health status and what you have done for it. The doctor will also ask you about any previous
injuries or accidents and how the rest of your life is being affected by your present dis-ease. Please feel free to be
open and honest with the doctor and address all concerns that you may have.
Depending on your specific health status, the doctor may feel that further investigation is warranted. If this is true,
these additional services will performed on this visit.
In order to help us get a better understanding of your current physical, mental and spiritual health, you will be asked
to complete some paperwork. Please allow 10-15 minutes for this. For your convenience, you can click on the links
below to print out and complete the forms prior to your appointment.
